All travel release forms require notarization

• General Travel Release Form

If you wish to travel to a country with a State Department warning or CDC Notice, you must contact one of the appropriate authorities below to obtain a special travel warning release form.  This form must be filled out and signed by you and a parent or guardian (if under 18) or next of kin.
